A rare caseof sporadic hemiplegic migraine with Hereditary Angioedema Type II (C 1q deficiency) diagnosed by senior Gastroenterologist Dr Amit Bundiwal in Indore. Now presented with recurrent vomiting and blood in stool.

Young pt from indore diagnosed with sporadic hemiplegic migraine with Hereditary Angioedema type II on tab Danazol and NSAIDs

Presented with recurrent vomiting for 2 months, last 15 days vomiting was bilious and blood in stool.

Significant weight loss around 20 kg.

Pt underwent investigation and UGI endoscopy. Endoscopy shows large gastric ulcer and duodenal ulcer diffusely oozing of blood like vascultis u lcer with tight duodenal stricture ? Steroid / NSAIDs induced.

Pt managed conservatively with blood and fresh frozen plasma.

Endoscopically APC (argon plasma coagulation) done to stop bleeding and balloon dilatation done for stricture dilatation.

Bleeding and vomiting stopped after Endoscopic intervention.
